Terms & Conditions for Fostering a Dog

I attest that the information provided on this application is true and accurate to the best of my knowledge.  I understand that completion and submission of this application does not guarantee that I will foster a dog from Protectors of Animals Inc.  I hereby permit Protectors of Animals, Inc. to contact my veterinarian and landlord if applicable. 

 

I understand that there is always an adjustment period for any foster dog coming into my home.  The average time a dog spends in a foster home can vary and I am willing to work to make this foster dog a member of my family during his/her stay.  I understand that Protectors of Animals, Inc. cannot be held responsible for the actions,  behaviors, and/or medical condition of all of the dogs in its rescue.  I agree to assume the risks implicit in working with animals that may have been abandoned, beaten or otherwise mistreated or abused, or who may suffer from a condition or disease.  Although Protectors of Animals, Inc., evaluates dogs prior to accepting them into its rescue they can not guarantee the temperament of any of the dogs since most of their histories are unknown.

 

As a foster you must agree that said dog will be kept on leash AT ALL TIMES when in an unfenced or unenclosed area – even if I am just walking from my house to my car.  All rescues are prone to being escape artists, especially in a apartment/home or when they are left alone.  They may bolt out of an open front door.  I will be aware and careful.

 

I agree that Protectors of Animals, Inc., reserves the right to check on the welfare of this dog on my premises.  I agree that said animal is the property of Protectors of Animals.  I realize that as a foster I will receive no payment for caring for said animal.  In the event. That I do not comply with the guidelines set forth by Protectors of Animals, Inc. the agency has the right to remove said animal from my home.

 

I understand that if, at any time in the future, I cannot continue in the foster program for ANY REASON, I will not sell or euthanize the dog.  I will contact the foster lead or kennel manager for arrangements to be made to move the dog(s) into another foster home.  I understand that Protectors of Animals, Inc., will be responsible for any expenses that may occur while the dog is in my foster care.  If a veterinary visit is necessary, please contact the foster team lead for authorization before taking the animal in for the visit. 

 

I have read and understand the above terms on this form in its entirety and understand the risks involved in fostering a dog.  I also agree that if the information given on this application is not accurate to the best of my knowledge, I forfeit all rights to any and all foster dogs residing in my care.
